COVID-19 Update & Waiver Form

Liberated Salon will be following all CDC guidelines for sanitation as well as taking measures to fight against the spread of COVID-19. We will be going above and beyond our normal high standards of safety and sanitation, as well as adding extra procedures. We hope that during this time we can work together to make Liberated a safe space for our clients and stylists. Everyone’s health is our main priority.

  • Our salon will be disinfected after every service and throughout the day. This will include stylists tools, chairs, armrests, shampoo stations, doorknobs, countertops, electronics and restrooms etc.
  • We are asking that any guest who is feeling ill or has been sick within 72 hours prior to their scheduled visit, reschedule their appointment.
  • All Stylists will be monitored for health and will receive a temperature check before entering the salon each morning. 
  • Stylists will be wearing protective gear during your appointment as well as sanitizing their hands in between each service. 
  • Once you have arrived for your appointment, please text your stylist, or visit the front desk from a safe distance to let us know you are here. Once your Stylist has completely sanitized their station and is ready to welcome you into the salon, they will contact you. In the meantime, feel free to wait in your car or take a stroll down Atwater Village.
  • Masks will be required upon entry, covering your mouth and nose.
  • Your temperature will be checked with a touchless thermometer; anyone with a temperature above 99.6 F will be sent home.
  • Wash and sanitize your hands before sitting down with your stylist, and put on a disposable cape.
  • Unfortunately, at this time there will be no snacks or beverages offered. 
  • Due to capacity guidelines we ask that you arrive to your appointment alone.
  • In order to keep the amount of people in the salon to a minimum, we will not be able to provide a service that is not previously scheduled. For example, if an appointment is made for an all-over color, we will not have time to accommodate a lengthier service, such as highlights. Due to sanitizing procedures, our time is limited and we will need to book additional services for a later date. It is the responsibility of the client to ensure the correct service is booked. Clients will be charged for all unfulfilled or incomplete appointments.
  • Please review our price list as some services have increased due to contributing factors.
  • Any appointment missed due to illness will be rescheduled once you are feeling better.
  • We ask that you arrive to your appointment in good health. As a team we will be taking proper precautions to ensure a healthy environment. Liberated Salon is our second home, one that we have enjoyed sharing with all of you.

Due to the global pandemic outbreak of COVID-19 and for the safety and protection of the health of our staff, stylists, and clients we are implementing additional safety measures in compliance with the safety precautions published by the Los Angeles Department of Public Health.

I confirm that I am not presenting with any of the following symptoms of COVID-19 identified by the CDC:

Fever above 99.6F

Chills or Body Aches


Sore Throat

Shortness of Breath or Difficulty Breathing

Loss of Taste or Smell

Flu-Like Symptoms or Runny Nose

I understand that for the safety of everyone, my temperature will be checked before the services are started. If my temperature is above 99.6 F, I agree to leave the salon and will not be rebooked for a minimum of 14 days.

The stylist and employees of Liberated Salon agree to abide by these same standards. We also affirm that we have improved and expanded our sanitation protocols to more thoroughly prevent the spread of COVID-19 and other communicable conditions. I understand that I must sanitize my hands before entering the salon and wear a mask that covers my mouth and nose. This mast must be properly worn during my entire salon visit.

Only customers receiving a service will be allowed in the salon. I understand I may NOT bring children or anyone else who does not have an appointment into the salon.

I verify that I have NOT returned to California from any country outside of the US in the past 14 days.

I understand that physical distancing of six (6) feet between the client and stylist may not be possible while in the salon receiving services. Our staff will maintain six (6) feet of distance between themselves and you, the client, as much as possible in our salon setting. We will also be requiring this same procedure for client-to-client contact in order to limit the exposure to all individuals.

I understand that Liberated Salon, stylists, staff, and all others will not be held liable for any exposure to COVID-19 or any other contagion.

I verify that the information I have provided on this form is truthful and accurate. I understand all the potential risks and complications related to COVID-19, and I would like to proceed with my desired salon services.

WAIVER AND RELEASE, I HEREBY WAIVE ALL CLAIMS AND FOREVER RELEASE Liberated Salon, including its officers, directors, independent contractors, employees, successors, assigns, agents, heirs, representatives, and volunteers as applicable, in their individual or corporate capacities, of all claims, liabilities, agreements, and causes of action of any nature due to any injury, loss, or damage to person or property, that may arise out of my services performed at Liberated Salon .

By signing below, I agree to the above statements and release Liberated Salon, its staff and practitioners from any and all liability for the unintentional exposure or harm due to COVID-19.
